submit the application to the PR-LSAMP Office is

TIPS to complete the
process:
- Use the NIST 2012
checklist to make sure that you have included
all required documents.
- Submit the
documents to the PR-LSAMP Institutional Liaison
Officer or mail it directly to the PR-LSAMP Main
Office at the address below.
- If you decide to
mail it, please send the application form and all
required documents to the following address:
