TIPS to complete the process:

• Use the NIST 2010 checklist to make sure that you have included all required documents.
• Submit the documents to the PR-LSAMP Institutional Liaison Officer or mail it directly to the PR-LSAMP Main Office at the address below.
• If you decide to mail it, please send the application form and all required documents to the following address:

• Students at UPR-Mayaguez may submit the application and all required documents to the PR-LSAMP office at the Resource Center for Science and Engineering, located in the first floor of the Physics Building.
